1.  Has a competent person for confined space, assigned in writing, evaluated the potential for permit-required confined spaces (PRCSs) using the procedures and decision logic chart of Figure 6-1?  (06.I.01a & b)



2. Does the assigned competent person meet the definition of “ Competent person for confined space” in Appendix Q? (06.1.01a)



3.  Is a list of confined spaces (permit-required and non permit required) maintained on site?  (06.I.01c)



4.  Have all permit-required confined spaces (PRCS) been identified with a sign?  (06.I.01d)



5.  Are confined spaces reevaluated at least annually or whenever they or their characteristics change in a way that could lead to reclassification as a PRCS?  (06.I.01e)
